Letter to President Trump

On October 1, 2025, 152 American healthcare professionals who have volunteered in Gaza since October 7, 2023 wrote an open letter to the White House about what they saw in Gaza, and why American policy in the Middle East must change.

Dear President Trump,

We are 152 American physicians, surgeons, nurses, nurse practitioners, physician assistants, and midwives who have volunteered in the Gaza Strip since October 7, 2023. Many of us have served on multiple deployments. Collectively, we spent more than 460 weeks volunteering in 22 of Gaza’s built and field hospitals, including all of Gaza’s largest hospitals. Our backgrounds include public health and humanitarian service in the world’s worst conflict zones, including Ukraine, Syria, Iraq, Afghanistan, Libya, Liberia, Chad, Yemen, Sudan, South Sudan, and Democratic Republic of Congo. We are civilians, veterans, and reservists, a multifaith and multiethnic group including Christians, Muslims, Jews, and others. None of us support the horrors committed on October 7, 2023, by Palestinian armed groups and individuals in Israel.

We are among the very few neutral American observers that Israel has permitted to enter Gaza for nearly two years. Given our broad expertise and direct experience working throughout Gaza, we are uniquely qualified to comment on several matters of importance to our government regarding American support for Israel’s ongoing attack on the Gaza Strip. We believe it is imperative that the United States immediately end its military, economic, and diplomatic support for the ongoing destruction of Gaza, and support an international arms embargo on all warring parties. This is the right thing to do, and we believe it is required under both American and international law.

Mass Violence Against Civilians

The scale of violence directed at Gaza’s civilians is unlike anything any of us has ever seen. Half of Gaza’s population is children, and we routinely treated them for every possible type of injury: 100% total body surface area burns, dismemberment, gunshots to the head and chest, evisceration, and even traumatic hemicorporectomy and decapitation. We have even seen these injuries in fetuses, whose mothers’ bodies could not protect their unborn children from the American-made bombs dropped on them by Israeli forces. Most of us saw pre-teen children shot in the head or chest on a regular basis, often multiple times per day.

President Trump, we wish you could see the nightmares that plague so many of us since returning from Gaza: scenes of malnourished children maimed and mutilated by our weapons, and their inconsolable mothers begging us to save them. Today we beg you to hear the cries of Gaza’s children that our consciences will not let us forget. We cannot fathom why our government continues arming Israel while its armed forces kill children en masse.

Those of us who have worked in Gaza since May of this year, when the Gaza Humanitarian Foundation (GHF) first began operations, have witnessed a new and shocking form of violence: the mass killing by shooting, shelling, and bombing of people seeking food for their starving families. We appreciate that your envoy Mr. Witkoff spent a few hours in Gaza on August 1, but we have collectively spent more than 460 weeks there. We cannot emphasize strongly enough: the GHF is a humanitarian abomination. It is a uniquely cruel tool of coercion that the American taxpayer should not be funding and that Americans should not be staffing. Two American contractors have described in detail their time working with the GHF, including observing Americans shooting Palestinian civilians indiscriminately and hiding evidence of crimes against humanity.

Whenever any GHF site opens, the day’s events are entirely predictable: within one hour the nearest hospitals will be flooded with hundreds of wounded aid seekers. Each one of these mass casualty events instantly overwhelmed the already massively overcrowded and under resourced facilities. Many patients are dead on arrival or have injuries that are beyond the capabilities of Gaza’s healthcare system. The wounded often clutched plastic bags under their torn clothing, bags they had hoped to fill with whatever meager provisions they could find. When we finished with the first wave of patients, we braced ourselves for the second and even third waves that were sure to arrive. None of these people were combatants. They were simply civilians seeking out food to feed their starving families. As the news media has reported, some of these aid seekers are even being killed by American subcontractors whose salaries and weapons ultimately come from the American taxpayer.

Sickness, exposure, and starvation

With only marginal exceptions, everyone in Gaza is sick, injured, or both. This includes every aid worker, every international volunteer, and probably every Israeli hostage: every man, woman, and child.

By early 2024 malnutrition was already widespread in Gaza, even among our healthcare colleagues. Since March 2, 2025 when Israel completely closed the border to humanitarian aid and commerce, malnutrition and deaths from starvation have skyrocketed. Today virtually everyone in Gaza is visibly malnourished. We watched our own colleagues lose consciousness at work from malnutrition and dehydration, eating virtually nothing as they gave all available sustenance to their children. Every one of us lost weight rapidly in Gaza despite having privileged access to food. We have photographic evidence of life-threatening malnutrition in our patients – especially children – that we are eager to share with you.

As in every famine, the first deaths have been in the weakest members of society: the poorest families, young children, sick children, widows, the disabled, and the elderly. As of today, medical sources in Gaza have reported 453 deaths from starvation, including 150 children. We beg you to realize that the number of starvation cases reported by the medical system in a humanitarian crisis is usually less than 10% of the true number of deaths caused by starvation. Without an immediate ceasefire to facilitate a massive influx of aid, deaths from starvation will increase rapidly in the coming months. The token gestures that Israel has taken in response to American and international pressure are entirely inadequate. Although the Integrated Food Security Phase Classification first declared Gaza to be in famine in August 2025 it is entirely possible that since October 2023 thousands or even tens of thousands of people in Gaza have died of starvation-related causes. Many if not most of them were small children. Families have turned to animal feed so that their children might survive, only to see the feed’s impurities kill their children by inducing liver failure.

With winter fast approaching, with potentially lethal levels of malnutrition affecting nearly everyone in Gaza now, and with most of Gaza’s population exposed to the elements, we worry that even more people will die this year of hypothermia than did last year. Again, most of them will be young children, and their deaths are entirely and easily preventable.

An astonishingly high percentage of our surgical incisions became infected from the combination of malnutrition, impossible operating conditions, lack of basic sanitation supplies such as soap or bleach, and lack of surgical supplies and medications, including antibiotics. Surgical success in Gaza was nearly impossible, leaving of our patients to die slowly of sepsis even if we could stop them from bleeding to death.

Malnutrition in Gaza is causing widespread spontaneous abortions and an epidemic of underweight newborns, while simultaneously making it impossible for new mothers to breastfeed. Gaza’s newborns are at incredibly high risk of stillbirth and death due to the lack of access to any reliable source of nutrition or clean water. In a shocking act of wanton cruelty Israel has even forbidden the importation of baby formula into Gaza, going so far as to confiscate it from the suitcases of international healthcare volunteers. We can never forget that the world abandoned these innocent women and babies to such a miserable fate.

The displacement of virtually all of Gaza’s sick and malnourished population, half of whom are children, to areas without running water or even toilets is disgraceful and shockingly inhumane. This has resulted in widespread disease and death from viral and bacterial infectious diseases, particularly in small children. Virtually every child under the age of five whom we encountered, both inside and outside the hospital, had a cough, watery diarrhea, or both. We found cases of hepatitis A in nearly every room of the hospitals in which we served, including in our healthcare colleagues. Epidemics are raging in Gaza today, and as with malnutrition, the most vulnerable and young children are the first to succumb to them.

Destruction of the healthcare system

The Israeli attack on Gaza’s healthcare system is simply unprecedented in world history. Israel has attacked every hospital of any significance and the vast majority of clinics in Gaza. Our healthcare colleagues have been murdered in cold blood with bombs and bullets, oftentimes within the halls of the hospitals where they spent their lives saving others. These are crimes of the highest order. Our own government’s support for them is intolerable.

Those of us who worked with pregnant women regularly saw stillbirths and maternal deaths that were easily preventable in any developing country’s healthcare system. The rate of infection in C-section incisions was astonishingly high. Women underwent vaginal deliveries and even C-sections without anesthesia. They were lucky to receive Tylenol afterwards, since no other pain medications were available.

Israel has destroyed most of Gaza’s healthcare system and killed more than 1,581 healthcare workers as of July 16, 2025. This is more than 17 healthcare workers killed every week since October 7, 2023. This is entirely unprecedented in the history of warfare. Several of the signatories to this letter were saved only by luck from severe injury or death during Israeli attacks on hospitals. Many of us wonder what the American government would think of American doctors and nurses being killed by an American bomb fired at them by an American ally. Would our deaths matter?

The hospitals where we worked were starved of the most basic supplies, from surgical material to ventilators to soap. They were regularly cut off from electricity and internet access, denied clean water, and operated at four to seven times their bed capacity. Every hospital was overwhelmed by displaced persons seeking safety, a constant stream of sick and malnourished patients seeking care, and a huge influx of seriously wounded patients.

Our Palestinian healthcare colleagues are among the most traumatized people we met. Like everyone in Gaza they have lost their own homes and family members. Our colleagues were acutely aware that their work as healthcare providers had marked them as targets for Israel. This makes a mockery of the protected status hospitals and healthcare providers are granted under the oldest and most cherished provisions of International Humanitarian Law.

Virtually every Palestinian healthcare worker has lived through or fled at least one Israeli siege of a hospital, as have many of us. The stories our healthcare colleagues told us about treating patients under fire, burying dead patients and even their own colleagues in mass graves, and surviving by drinking and washing with intravenous fluids seem to eliminate any sense of justice from the world. Many of our colleagues were taken by Israeli forces during these attacks. They all told us a slightly different version of the same story: in captivity they were barely fed, continuously physically and psychologically abused, and ultimately dumped naked on the side of a road. Many told us they were subjected to mock executions and other forms of mistreatment and torture. Far too many of our colleagues told us they were simply waiting to die. Hundreds of our colleagues remain in captivity. As of today, the director of every hospital north of the Netzarim corridor has either been killed or arbitrarily detained by Israeli forces. Some have been released without charges, trial, or even an accusation of wrongdoing.

Every medical school in Gaza has been physically destroyed. None of them were destroyed during fighting between the warring parties: all were systematically leveled by American-made bulldozers and in controlled demolitions. The medical students we taught in Gaza were some of the most dedicated and enthusiastic young people we have ever worked with. Many of them have been killed by Israeli forces. Seeing them cut down day after day with weapons we pay for has left many of our souls broken and empty.

The 152 signatories to this letter spent more than 460 weeks inside Gaza’s hospitals and clinics. We wish to be clear: not once did any of us see any type of Palestinian militant activity in any of Gaza’s hospitals or other healthcare facilities. We urge you to see that Israel has systematically devastated Gaza’s entire healthcare system and targeted our colleagues for torture, disappearance, and murder on an unprecedented scale, not because it was militarily necessary but in order to destroy that system’s ability to save lives.

 

Mr. President, our direct observations and many publicly available studies lead us to believe that the death toll from this conflict is likely many times higher than the partial reporting of violent deaths from the Ministry of Health in Gaza. We also believe there is probative evidence of widespread violations of American laws governing the use of American weapons abroad, and of International Humanitarian Law. We cannot forget scenes of unbearable cruelty directed at civilian men and women, and especially at children. Our government is a direct participant in these crimes. The American contractors who have worked at GHF sites may have participated in crimes against humanity.

What is happening in Gaza is not a war. In war, children who are still learning to walk are not regularly shot in the head. In war, homeless, starving civilians are not enticed into death traps using rice and lentils as bait. In war, two dozen doctors, nurses, paramedics, rescue workers, and firefighters are not killed every week for nearly two years straight.

Mr. President, any solution to this problem must begin with an immediate and permanent ceasefire in Gaza. The United States does not need to ask Israel to agree to this: we can impose a ceasefire on the warring parties by simply stopping arms shipments to Israel and announcing that we will participate in an international arms embargo against Israel and all Palestinian armed groups. We stress what many others have repeatedly told you, and what we told the previous administration: American law is perfectly clear on this matter, continuing to arm Israel is illegal.

We urge you to immediately withhold all military, economic, and diplomatic support for the State of Israel, and to participate in an international arms embargo against Israel and all Palestinian armed groups until a permanent ceasefire is established in Gaza that includes the release of all Israeli and Palestinian captives and hostages. These policies should remain in place until a permanent resolution to the Israeli-Palestinian conflict is negotiated and implemented between the two parties.

President Trump, we have witnessed crimes beyond comprehension. Crimes directed at children and pregnant women, at the weak and sick, at the hungry and desperate. We cannot believe you wish to continue facilitating this shocking assault on our collective humanity.

Please meet with us to discuss what we saw in Gaza with our own eyes, and why we feel that American policy in the Middle East must change drastically.

At a minimum, we recommend the following steps be taken urgently:

1.     All crossings between Israel and Gaza, as well as the Rafah Crossing between Gaza and Egypt, must be reopened immediately to allow unfettered aid delivery by recognized international humanitarian organizations. This must include not only food, water, and medical supplies but also temporary shelter. The Gaza Humanitarian Foundation should be defunded immediately, and the people who led it should be investigated for violations of American and international law. Security screening of aid deliveries must be conducted by an independent international inspection regime instead of by Israeli forces. These screenings must be based on a clear, unambiguous, and published list of forbidden items, with a clear independent international mechanism for challenging the prohibition of items.

2.     A bare minimum of 15L of potable water per person per day – the Sphere Handbook minimum in a humanitarian emergency – must be allocated to the population of Gaza.

3.     Full and unrestricted access of medical, surgical, and public health professionals and medical and surgical equipment to the Gaza Strip must be resumed. This must include items taken in healthcare professionals’ personal luggage to safeguard their proper storage, sterility, and timely delivery. All equipment needed to properly rescue people dying of starvation – from laboratory testing equipment to nasogastric tubes to enteral feeding formulas and ready-to-use therapeutic foods – should be prioritized. Incredibly, Israel continues to block healthcare workers of Palestinian descent from working in Gaza, even those who are born American citizens. Israel also arbitrarily denies the entry of more than half of international healthcare volunteers to Gaza. This makes a mockery of the American ideal that “all men are created equal” and degrades both our national ideals and our profession. Our work saves lives. Our Palestinian healthcare colleagues in Gaza are desperate for relief and protection, and they deserve both.

We are not politicians. We do not claim to have all the answers. We are simply healing professionals who cannot remain silent about what we saw in Gaza. Every day that we continue supplying weapons and munitions to Israel is another day that men and women are shredded by our bombs and children are murdered with our bullets. Another day that children starve in their helpless mothers’ arms.

President Trump, we urge you: end this madness now!
